Registration Forms

We welcome you to our Faith Formation programs for Sacred Heart St. Francis and St. John the Baptist Churches! You may register your family on one form. Registration for your first child is at the beginning of the form. Additional children can be listed at the end of the form.

We ask that a registration fee of $20 per student or $50 per family be submitted. Payment can be made:

On-line by clicking the donate button and choosing "Class Registration Fees".
In person at on the first day of class
Mail to: Sacred Heart St. Francis de Sales, Faith Formation Registration, 238 Main Street, Bennington, VT 05201.
Please do not let inability to pay the fee stop you from participating. Contact the office if you need a scholarship

**First Eucharist and Confirmation students who were not Baptized at Sacred Heart St. Francis of St. John the Baptist church will need to provide a copy of the child's baptism certificate.
